Thanks @HSESchools for telling 4th graders to experiment w/ anal sex. I wouldn't be putting this together otherwise.
Requirements for pulling your kid from the government schools are telling the principal you are withdrawing your kid, but if you don't send a certified letter you run a real risk of having a truancy cop showing up at your door...

You do NOT have to register w/ the state DOE. DO NOT REGISTER WITH THE STATE! Anyone telling you different is lying or ignorant. Either way, don't take advice from that person...
In Indiana you are required to provide equivalent instruction, maintain attendance record, and have 180 days of instruction(or the same as your local gov school.) Only the superintendent can ask for your records. DO NOT GIVE THEM TO ANYONE ELSE!...
It's unlikely that you will need legal help, but it's smart to have some insurance. @HSLDA is a lawyer on your side for $10 a month. On top of that, they lobby for legal protection for home educators...
Remember, these are your kids. Their education was always your responsibility, but you are not alone. I like @sonlight for a core curriculum. It's literature-based. We supplement and sometimes trade out some books, but it's a great system to build on...
If you are not a math person, consider a computer-based program. We like teaching textbooks for math, but in addition to that we use "Life of Fred."
