Here's my idea

Inko was originally pregnant-

-with twins, boy and girl. But later into the pregnancy ultrasound reveals the boy had been absorbed by the girl. Heartbreaking for poor Inko but its not uncommon so time passes and Inko gives birth to a healthy baby girl she names Izumi and whispers the boys name, Izuku.
