Pancreatic cancer stroma remains a dilemma for physicians scientist! In our study with @EileenMOReilly and Gokce Askan we identified that patients with dense stroma had lower cumulative local recurrence compared to pts with loose stroma!

Even though we haven’t seen statistically significant difference in OS outcomes between subgroups, pts with dense stroma had favorable outcomes. Most notably PDAC cancer stem markers were more pronounced in pts with loose stroma!
Collectively, these findings are consistent with growing evidence from clinical trials which intended to deplete dense tumor stroma and failed show any benefit and perhaps seen adverse outcomes! These growing evidence suggest that PDAC stroma is more sophisticated then we think!
Therefore “depleting dense stroma” does not appear to hold future promise... Although our findings are correlation with prospective clinical studies, larger cohorts are needed to confirm our findings !
