I thought to write a thread, looking at how things are in India add impending doom for us, our model of fighting COVID-19 needs to be tailored to our unique problems.

We first need to rapidly assess our short comings and expected bottlenecks/limiting factors. 1/n

and then quickly establish high impact points and actions that will use least resources and more impact on how we look when we get out of this.

Our population to Healthcare professionals and critical care beds ratios are abysmal. 2/n

Critical care will be a luxury when we are the eye of Covid-19 storm. There is no need to spend on it with survival benefit to resources spent not in good range.

1. As we stand in my opinion, we need to maximise efforts to establish seamless supply chains and reserves. 3/n

2.Oxyegen is of prime importance. In India we saw how quickly it became a limiting factor to the effort against covid. This the time to not only establish apparatus for Oxygen plants but also storage and distribution.(on emergency basis) 4/n

3. Important medications like Blood thinner injection, steriods, antivirals and anti biotics need to be accumulated ASAP.

4. Hospital space will become prized. There won't be enough beds. Rather than patients lying on roadside with Oxygen cylinders, teams of healthcare... 5/n

.. Staff need to be trained urgently in use of equipment at people's home. To avert admissions. This model is successful in averting admissions in Developed world under Acute community teams that are lead by doctors who provide Intravenous meds, Oxygen and reviews at home. 6/n

5. There is also a need to get information on the number of stocks and oxygen supplies in realtime so that efficient and flexible transfer of resources could happen between Major hospital at Provincial and National level. There needs to be perfect coordination here. 7/n

6. Send Covid-19 emergency messages to interrupt usual TV and Radio transmissions to create Alarm/ awareness about Covid-19 and reduce desensitisation.

Only through foresight, cooperation, just use of resources and intelligent Governance will we be able to make through this.8/n
