3/ The open source, open government movement g0v (gov-zero) started in Taiwan is a great inspiration for this.
By creating "forked" versions of governmental websites and processes these can be co-developed with closer connection to its users, the citizens.

4/ I believe that some governmental urban planning documentation can be developed and worked on through GitHub repositories.
I'm yet to find any examples of this. Although, I hope that citizens, in a near future can commit and participate in such urban planning repositories.
