I recently noticed that I was "following" users i don't recognize. If you've used 3rd-party tools that analyze your circle of friends, you might notice the same thing. You might want to review "connected apps" in your settings & revoke access from suspicious ones.

+I believe the "circle of friends" tool was modifying my friend list. Looks like the app accessed my account just days ago, when I haven't used the tool for months. If you've used it, you might want to revoke the access the app has to your account using the steps in this thread.
