The 3rd wave is maturing and I hope we pass the peak in the next few days/weeks. In some areas such as Qom we have already passed the peak while in some areas such as Khuzestan the wave is behind Tehran and Qom. Hearing from my families, situation is DISMAL.
While COVID has completely ravaged the country, it has been 8 months since the Supreme Leader has had a meeting with the heads of state - possibly in the fear of catching the virus. This ended yesterday and the topic was COVID - showing that...
... this virus along with the financial situation Iran is can result in a complete meltdown of the fabric.

Overall, from all accounts (official, anecdotal and witnesses/my contacts) the criticality of the situation cannot be overemphasised. COVID is using up all medical system resources, bills piling up, the staff tired and fed up and the hospitals on the verge of bankruptcy.
Every single day there is a new sit-in and protest by nurses fired or the COVID salary extras being overdue for months (6 months). The financial system already on the verge of collapse with inflation 40% - people are pissed and desperate.

The question is when and not if.

While numbers in Qom are going down rapidly, there are news of lack of hospital beds in Khuzestan province (4.7M). Khuzestan had a terrible 2nd wave and was not affected by the 3rd wave - only that it has been now and showing stress signs. https://twitter.com/dw_persian/status/1320303857575038977?s=20
The situation in Tehran is still critical with complete lack of empty ICU beds.

In W Azerbaijan, authorities have asked permission for a 2-week lockdown due to high number of cases.

Anyway, you get the picture: Iran without much doubt is the world's worst impacted country on earth.

But as I said COVID is one of the worries. The financial meltdown is terrible pushing people to despair, aggression and suicide.
And if that was not already enough, there has been 3 case of police brutality just over the last 3 days.

This disturbing footage is one of them and the victim died possibly from excessive pepper spray.
