They got away with it, because they were meant to be played at a tournament, ie. a nerd convention. You were playing with five or six strangers you'd never see again, so who cares.

I mean, White Plume Mountain was basically just a dark murder version of Takeshi's Castle, with players exploding in time to Souza music. It's literally where "rocks fall, you die" comes from.

I think it's right at the entrance, too. First PC to walk through that doorway, the module just says something like "a slab of granite the size of Vermont falls on them. Don't even bother fucking rolling for damage."
