I just want to say that I have great insurance that I pay a lot of money for, and I still spent over an hour on the phone today explaining to them that the X-ray I got last month is covered because X-rays are SPECIFICALLY AND UNEQUIVOCALLY COVERED by my insurance.
Honestly, it was mind-blowing. Here's a verbatim excerpt:

CIGNA: X-rays are not covered by preventative care.
ME: Okay, but they ARE explicitly covered by my plan.
CIGNA: Where are you seeing that?
ME: Page 3, paragraph 2 of my EOB, where it says "X-rays"
CIGNA: ... Ah, yes.
Anyway after literally a full hour, during which time they called the hospital that did the X-ray and the doctor who requested it, they informed me that they would resubmit the claim with a note to pay attention to the part of my plan that clearly covers X-rays.
They also said that there was no guarantee it would be covered, and could take up to a month to figure it out.

Private insurance is a massive SCAM.
The only people who "love" their insurance are people who have never had to use it.
