Ours is pretty simple (I coach 2 teams back to back, so often the players run the warm up).

We want to engage them in the decision making process (4v2s instead of just technical actions) as well as prepare them psychologically (as @DanAbrahams77 says, physiology dictates psychology) and they have to be able to perform maximum explosive sprints by end of warm up.
Two 8x10 grids (3-4 yards apart), players will use the same grids for 4v2s and movement prep. Activate muscles, then stretch dynamically.

Set 1
Activation: Jog/Skip/Shuffle/Karaoke
Dynamic Stretch: Quad stretch/Lunge (side + forward)/ Elbow to instep
Playing Style: 4v2 (2 min)
Set 2
Activation: Back Pedal/Butt Kicks/Bounds
Dynamic Stretch: Knee Hugs/Leg Cradle/Hamstring Sweep/Straight Leg Kick/ SL RDL
Playing Style: 4v2 (2 min)

Set 3
Activation: 75%/85%/100% Build Ups
Dynamic Stretch: Calf Stretch
Playing Style: 4v2 (2 min)
From a psychological perspective, I challenge both groups to be louder than the other when counting passes in the 4v2 and award a winner at the end.

This gets them out of their shell and communicating with their teammates.

This part takes 10-12 min.
I then give them the line up and tactics. Players who are not starting are neutral players in possession game for 4-8 min (back 4 + holding mid together as a team vs front 5). 10 min prior to kickoff I give them space to do what they feel they need.
Right before kickoff (and after halftime) we do 3 short sprints from the sideline to ensure we are ready to go!
