Pick your poison when it comes to biomarkers to follow. Soluble IL-2 has best correlation with PET-FDG activity. Mine is CRP.

No specific recommendation for serial CT or PFT. However, watch out for that 26% that may have stable PFT with worsening symptoms and CT scans.

You can choose elaborate predictive models like Walsh model or GAP model. If not, watch out for Pulmonary HTN or significant fibrosis as both associated with increased mortality. #CHEST2020 #sarcoidosis

The challenge is deciding when to treat The asymptomatic patient. A steady decline in PFTs / worsening radiology may warrant treatment. Otherwise, remember the poor predictors and discuss treatment with your patient.
