Killua and Alluka are closest in age. The Nanika incident happened when she was five and when Killua was six. Incidentally, Killua was also six years old when he was first sent to Heaven’s Arena. They only played with each other, because Kalluto was a toddler and much too young.
They sent Killua away for two years, and most certainly stuck the needle in his head before he left. If they sent Illumi after him when he was twelve for supervision im the Hunter Exam, there were ABSOLUTELY drastic precautions taken before he went to fight in Heaven’s Arena.
There’s two aspects that this needle would serve. First and foremost, not fighting anyone stronger than him, the purpose of which is obvious considering that he was at an arena where death matches are common.
Second though, subconscious obedience to his family, which manifested in the repression of his memories of Alluka. In the two years that he was gone, Silva was using Illumi to test out Alluka’s powers. Something to note is that they sent killua away AFTER he explained her powers.
Killua’s absence for two years ensured that no one would be there to protect Alluka. Kalluto was two years old when Killua left: he never knew Alluka as a person. His family taught him to fear his sister, to see her as a monster. Milluki was cruel. Illumi was too far gone.
As a result, Kalluto never ever got to have a healthy sibling bond the way that Alluka and Killua got to have. He was the baby of the family, and for two incredibly formative years he was basically stuck with his mom and two preteen/teenage brothers.
When Killua returned at age eight, he was not the same Killua who had left. Heaven’s Arena is somewhat of a coming of age ritual for the Zoldycks: illumi went through the same process. I suspect that when they reach the 200th floor, they are deemed ready to go on actual jobs.
So as soon as he came back, Killua was sent on jobs. Kalluto went through training on his own. He had an incredibly lonely and isolating childhood. Killua was someone he looked up to and desperately wanted attention from, but never got what he wanted.
So when Killua ran away and stabbed his mother and Milluki in front of Kalluto, it was an awakening for him. This, combined with seeing Gon and co. trying to rescue Killua, showed him that a world outside existed, and he’d have to chase killua in the wild.
In the 1999 anime, Gon is chased my Mike. Seaquant, the assistant gatekeeper, says that only a Zoldyck can stop Mike. When Gon is about to die, we hear a whistle. Mike runs off. Kalluto is shown. He deliberately saved Gon’s life by calling Mike back.
In the Chimera Ant Arc, he says that he joined the Phantom Troupe to save his brother. It’s clear that Killua is the brother in question. Kalluto, thought of as obedient and shaped by his mother, actively rejects his family’s orders because of his love for Killua.
I wonder if Killua is at all conscious of how much Kalluto adores him. Kalluto symbolically gave up his role as a Zoldyck by joining a troupe run by one of his father’s greatest adversaries. Troupe members are loyal to the group first and foremost by default.

Kalluto had no one. Even Illumi has hisoka. When he joined the Troupe, he finally had a group that saw him as his own person and respected him and loved him. He finally found a family that loves him and now has to watch them die, one by one.
