(1/5) Hey Ohio, you've heard about these "naked ballots" but you have questions? We have answers.
(2/5) When voting by mail, when you return your ballot, all highlighted spaces on the sleeve envelope MUST be filled out correctly. If information is missing or incorrect, your county board of elections may not count your ballot and your vote won’t count.
(3/5) Here is a picture of the ballot sleeve. If it is not filled out completely, the voter will be sent an 11-s form.
(4/5) If you do not give complete or correct information, you can still have your voice counted by giving supplemental information through the 11-s form by Nov. 10th.
(5/5) Provide the missing information & return this form to your county elections office. You can:
•Deliver the completed form to the county board of elections office by 11/10
•Mail the completed form so it is postmarked by 11/10 & received at the board of election by 11/13
