The president is on dexamethasone, a powerful corticosteroid, which is used to minimize the inflammatory portion of the immune response in more severe COVID-19. However, giving it now, 2-3 days after antibody therapy, seems to me a poor choice. Any immunologists want to weigh in?
My thinking is that the dex will impair the antibody response of the president necessary for the period 2-3 days after the antibody therapy.
So, dex has a potent effect to reduce circulating immunoglobulin levels. Thus impairing response to the virus. It has beneficial anti-inflammatory effects later in the disease. Something here really does not add up.
As a further thought, the monoclonal clearance half-life is VERY long, around 21 days. So perhaps you give dex now to improve condition and are confident it will be gone long before the monoclonals are gone? Seems a lot of speculation.
