So, my brain is really similar to ADHD and if you're like Crom or me, there are some things (call em hacks if you want) you can do to make use of your brain's particularity. First thing for me is keeping your projects modular. One module can be the vision and general overview for what you want to create. That's fine. Stick to broad strokes and document everything. Carry a notebook with you for when the ideas hit, you need to capture them.
Following modules should be concise and easy to grasp, again document them. The idea is that when your passion invariably drops, you have captured your ideas and can revisit them later.

New projects come up. Don't fight them, most of all don't worry about finishing things.
When you have a burst of ideas you can use for one of your ideas, come back and integrate them. You can take the time to merge several small modules and make them a cohesive whole, but the idea is to be satisfied with your progress on small tasks. And document everything.
Integrating several separate modules requires understanding the needs of individual projects, but you can really benefit from joining two ideas that came out of distinct realms.
Also if you're ok with teams, try finding a partner that shares your love for what you do, but is otherwise DIFFERENT from you. Like, get an organized, calm and diligent friend on board, and you will be able to assemble projects from modules.
The most important lesson is to stop feeling guilty and unproductive. Your neurotype is what it is. Might as well embrace it and not feel bad about not being neurotypical.
