Good evening #medtwitter! I do a thingy w/ my pts that I figured was pretty standard, but one of my pts this week told me to share this thingy w/ others if possible. So, just in case you aren& #39;t already doing it, here goes... 1/
When I review a pt& #39;s meds w/ them, I ask them what each is for, tell them if they don& #39;t know, then ask them if it aligns w/ their goals, & if it& #39;s helping to achieve said goals... 2/
If it does and it is, frikin& #39; awesome! & I review how to take it properly to make sure we are achieving said goals the best that we can. If it doesn& #39;t and isn& #39;t, not awesome! & we talk about DC-ing that med... 3/
If it& #39;s a med out of my lane, then I encourage them to ask the prescriber about it, & often times I& #39;ll message them / cc them on my note to explain my thoughts & support the pt asking them about it, cuz what are we doing if not collectively helping our pts achieve their goals? 4/
That& #39;s really it, it& #39;s a pretty simple thing to do, but it may make a heck of a lot of difference to a patient, like it did for mine this week.
