I'm surprised to see that "Psychiatry" is considered very mediocre, deglorified subject. Yes, the layman will not understand but even the medical professionals are decrediting us.

One of my clients, whose cardiac pathologies were ruled out, CT angio also been done and put on Paroxetine by me was advised to stop it, that i had instituted for panic anxiety, by a renouned cardiologist.

Ridiculous, he should mind his business. Who knows psychiatry better ???
Another #client with #anxiety, predominantly SOB, was advised ECG, ECHO, Holter, TMT, all being normal. Further he was advised CT angio, to rule out PE. Again risiculous. Does PE not have clinical features???
His gurdian ridiculed the physician and ask for #psychiatrist consultation, which they were denied.

Later I treated him with Escitalopram for 6 months and now he is off his treatment. Perfectly normal......
If you see situation of big hospitals, psychiatry is not considered a speciality at all.

#Civilhospital has massive number of patients in psychiatry OPD, but not a single "full time psychiatrist",
Many consultation-liasions but no inpatient.
Oldest hospital, Bir has just started part time #psychology & #psychiatry OPD. Again, no inpatient services.
There's only one tertiary " #mentalhospital" throughout the country, that too in a pathetic condition. Allotted 50 bed, 47 only functional, always occupied.
If any organicity need to be ruled out, need to refer to #PAHS.
The only #mentalhospital don't have #neuroimaging services. "Serum lithium" level started after 35 years of establishment.
No cabin or delux, if patient of upper middle class want treatment.
So where are we lacking ?? Is it we as a mental health professional have failed or the system???
We don't have a mental health policy till date.
And everyone uses #Nexito, because medical representatives have taught them, but do they actually know how is the dose titrated, how long it is meant to use and how it is tapered.
Sometimes the client gets better, you continue antidep as long as clients don't ask why and what for are they continuing this particular medication. If they don't improve, condition gets severe then you refer. Well played...
