*In the Unfortunate event of a Death of a Corona Warrior / Accidental death on Covid duty (w.e.f. 31st March 2020 to 25th September 2020 presently), there is a provision of 50 Lakhs compensation under the PM Garib Kalyan Insurance scheme*

It has come to notice of ICHA Mitra Team that NOK of deceased are not guided in any office as to how to put up for claim.
You are REQUESTED to widely circulate in the groups so that NOK of the deceased Corona Warrior is Helped. They can be referred to http://www.ichamitra.org  too

Documents to be submitted In case of Loss of life due to COVID19 / Accidental death while on Covid duty, following documents are required:
*1. Claimant needs to fill up claim form along with necessary documents as prescribed by New India Assurance Co.

(Available on http://www.ichamitra.org  site) and submit the same to Healthcare Institution/ organization/ office where the deceased was an employee of /engaged by the institution.)*

*2. Identity proof of the a. Deceased and b. Claimant (Certified copy)*

*3. Proof of relationship between the Deceased and the Claimant (Certified copy)*

*3. a. Bank details of the claimant with cancelled cheque.*

*4. Laboratory Report certifying having tested Positive for COVID-19 (in Original or Certified copy)*

*5. Death summary by the Hospital where death occurred (in case death occurred in hospital) (Certified copy).*

*6. Death Certificate (in Original) by Municipal authority.*

7. Certificate by the Healthcare Institution/ organization/ office that the deceased was an
employee of /engaged by the institution and was deployed/drafted for care and may have come in direct contact of the COVID-19 patient. For community health care workers,

the Certificate should be from Medical Officer of Primary Health Centre (PHC) that ASHA/ASHA Facilitator was drafted for work related to COVID-19.

*The claim is then processed by the designated nodal officer in each State/ UT and sent to Insurance Co. for further processing.*
