The night SHO has accepted a pyrexial flank pain straight from ED without urology review.
“So you added on a D-dimer for this patient who’s on treatment for myeloma and has had five previous VTEs?”
The consultant is doing a round robin for causes of pericarditis. He is second in line and the FY1 has already taken ‘idiopathic.’
The gastro consultant pops in. He thinks they’re friends since his last rotation.
“The patient is for resus but not for ICU? Tell me more.”
Phone call just in. The day reg is sick but the other SHO is happy to step up and take the bleep.
“No-one has thought of sarcoidosis here? Say whaat!”
The patient has an incredibly rare syndrome he has never heard of.
The night SHO thought the PR could wait until the morning. That’ll be his job now.
Sounds like there is a ?cord compression in his bay, which means he’ll have to perform a full neurological exam and speak to radiology for an MR spine.
“You’re surprised they’ve not cardioverted with digoxin.”
“So they were apyrexial, the chest x-ray was clear and inflammatory markers normal but yet you have prescribed co-amox.”
The night SHO is relaying a detailed family history for a patient with NSTEMI.

The night team explain it wasn’t possible to do the LP for a patient in his bay overnight. Besides, the lab won’t process xanthochromia out of hours, even in this tertiary neurosurgical centre.
There are still 25 patients to be seen.
Explaining that there were 5 crash calls and a 6am flurry of patients last night. His response...
The night SHO started a patient on SC heparin for PE when they would actually fit criteria to be ambulated in Apixiban.
A patient he saw yesterday has been transferred to ICU overnight through no fault of his own. Everyone is trying to be supportive.
“Dwayne, did you prescribe this furosemide yesterday? It’s just their U&E have totally gone off.”
