3/ to be explained, in order for the arguments to be understandable by professionals in health care and related fields, who are often unfamiliar with many of the relevant details.

JAMA said they were not accepting any commentary due to high volume.
4/ MedScape contacted me and offered to published these ideas as a Perspective. I am told that MedScape is well respected by people in health care.

Hopefully it'll help stimulate debate. Many thanks to @linseymarr and @kprather88 for reviewing a draft...
