Total failure of services. Bravery had a torrid time in a Priory ATU and had nowhere to live when he was discharged. He and his dad lived in cheap hotels.
And he was allowed out alone after telling his carers he wanted to kill someone.

And why @CareQualityComm did you award his care provider a ‘good’ rating with a ‘good’ for safety after this happened?
