I'm thinking about DevOps today, and want your input.

Q1: Have you ever worked as part of a DevOps team, where you both participated in developing features and in supporting the running service in which those features shipped?

Q2: When a problem caused a live-site incident, did your team's response focus solely on the immediate symptoms or did it include identifying and resolving the root cause so that there should never be another customer-impacting incident of the same type?
Q3: What team culture and priorities led to the response you selected in Q2?
