Excisional biopsy of a http://5.mm , long standing lesion in the lower eyelid of a 70 year-old female. There is no connection of the lesion with the epidermis.
Higher power view showing sheets of tumor cells with glandular spaces containing basophilic mucin. Some examples of these neoplasms can have papillary and cribriform architecture.
The tumor cells are relatively monotonous with moderate amounts of cytoplasm scanty cytoplasm, round to oval nuclei and inconspicuous nucleoli.
IHC showing strong but patchy staining for synaptophysin (left) and chromogranin (right).
DX: Endocrine Mucin-Producing Sweat Gland Carcinoma (EMPSGC). EMPSGCs are rare low-grade adnexal carcinomas with endocrine differentiation usually affecting lower eyelid. EMPSGCs are indolent neoplasms with low recurrence rate and only one reported case of lymph node metastasis.
