Was chatting with someone who is ex-Amazon who told me about how for every high severity incident, they have to write a COE (correction of errors) doc with the 5 whys, which then gets reviewed/people get interviewed by a “bar raiser” to root cause “cultural/systemic” problems.

I knew about “bar raiser” interviews ... but “bar raiser” post mortems?

That’s news to me.
