I've previously stated my critical view of @fordnation, @celliottability & @ONThealth prior (see tweet).

Here, I'll disclose how they threaten the care provided by specialists for Ontario's most vulnerable & complex patients

(SEE THIS THREAD) https://twitter.com/NeilMaha/status/1253344792886591494?s=20
Prior to March, @ONThealth acknowledged & agreed that specialists that provide complex care for these patient populations will continue to receive funding to provide this care via virtual means so that #PhysicalDistancing could be adhered to.
Specialists such as #respirologists #pulmonologists #geriatricians #hematologists #endocrinologists #allergists #rhematologists #pediatricians #neurologists recognize the vital importance of this as the patients we treat are complex & at high risk due to their diseases
Given it was agreed upon, we reached out to our patients to provide care during #COVID19. We did so to keep our sickest patients safe; away from hospitals. Patients on immune suppressing medications, oxygen, complicated therapies or advanced needs for care, etc.
Unfortunately in April, @fordnation & @celliottability decided that this care was no longer needed. They felt specialists shouldn't be funded to provide complex specialty care.

Yes. That's correct. The most complex patients had their funding withheld to their specialists.
Think about this. My #pulmonaryfibrosis patient on oxygen doesn't have funding for me to provide care to her.

The complicated #diabetic patient with an #insulinpump no longer has funding for their #endocrinologist to treat.
The elderly #Alzheimers patient who is seen by their #geriatrician has also had their funding withdrawn.

The pediatrician looking after a child with #autism has the funding revoked.
The list goes on...and on

How does this make sense you ask? Easy.

It doesn't.

The sickest patients need their specialists. We know this. @ONThealth refuses to recognize this
It is deeply concerning during a #pandemic that our sickest patients are left out like this. Specialists are reaching out to patients to bridge this gap regardless but, I'm concerned that it won't last.

We need to care for everyone. Especially our sickest.
Our patients deserve to hear from @celliottability @fordnation @ONThealth as to why they've excluded the most vulnerable.

To leave them forgotten exposes them to unnecessary risk and harm. I don't want that for them. Nobody does.

They need to hear from you.
This important problem has been on my mind for weeks. Finally, when on call today, caring for one of our sickest #covid19 patients, I couldn't help but think how wrong this is and how we must change this now.
Hard to imagine providing good care when @ONgov :

1) Fails to protect HCWs with adequate #PPE

2) Delays funding @OntariosDoctors to screen patients for 3 mos

3) Delays funding @OntariosDoctors for virtual care for 3 mos
4) Forces liability onto @OntariosDoctors unilaterally with no explanation of terms. Closures of hundreds of clinics are now threatened.

5) Renege on payment to specialists for caring for our most vulnerable patients.
