a sign that one's Ramadan was not accepted is that a person returns back to who they were before Ramadan began.
They leave Ramadan with the same bad behavior and habits they entered the month with. No improvement, no increase in Taqwa (the entire goal of fasting), nor desire to practice Islam the way it was revealed, no change to their daily schedule,
no extra prayers, no time spent with Allah's Speech reflecting over its meanings, no increase in good deeds on a daily basis, no increase in obedience to the parents, no extra fasts, no increase in charity, etc...
The major reason for this is because they were not sincere in their actions in the first place.

When they fasted, prayed, gave in charity, displayed good manners, secluded themselves in Itikaaf, etc. it was not for Allah's sake.
Instead, they did it for others to see and sought out their praises and good thoughts. As a result all of their "good deeds" were rejected and they return back as they were before this blessed month.
Surah Fatir 35:10, Tafsir ibn Kathir
The Messenger of Allah (ď·ş) said, "Almighty Allah says, 'I am the One Who is most free from want of partners. He who does a thing for the sake of someone else beside Me, I discard him and his polytheism." [Muslim].
Another reason for the rejection of their Ramadan is their disregard and lack of interest of performing these deeds in accordance to the Sunnah of Prophet Muhammad (pbuh)
the Prophet ď·ş said: "Whoever does not leave false speech, and acting according to it, then Allah is not in any need of him leaving his food and his drink."

Jami` at-Tirmidhi, Vol. 2, Book of Fasting, Hadith 707
