With no end to the COVID19 menace in sight at least in Maharashtra and Mumbai for the next few months, doctors cannot indefinitely stop routine work. Also given its massive sub clinical reservoir, the virus is likely to stay in environment long after this pandemic is over. 1/4
Which means we have to learn to live with it till a good vaccine is available: that is at least a few years from now.
Therefore it is probably better to discard fear and start seeing regular patients again, but with new norms for a new age.

Of course it goes without saying we would have to use PPEs all the time whether in OPD, OT or rounds.

This is what I would suggest:
Go to fewer places,
spend longer hours and spread patients out,
club indoor rounds and OPD together to minimise requirement of PPEs,

Also keep surface disinfectant at hand to apply on chairs and examination bed after every examination.
Lastly,
We will have to charge significantly more than what we are doing to accommodate additional cost, lower workload and higher risk.
