Lean and obese alike in this small human study had higher hepatic DNL on a HCLF diet.
On a LCHF diet, lean and obese normoinsulinemic had low DNL. Obese hyperinsulinemic still had high DNL even on LCHF!

Interestingly the recent study at the top of this thread found different fat composition in VLDL-TG as opposed to intrahepatic fat via MR spectroscopy while this older paper with different methods found the composition to be indistinguishable.
