남 = male, 여 = female
you can write members order from oldestyoungest or vice versa.
DAUM ID = Daum Email w/o @ daum . net
last Q just answer with number/add 개 (e.g. 6개 = 6 pcs)

you will notified if your form accepted and your status will change from 준회원 to 정회원
Rejected form won't be notified by the system. You can check if you get rejected either bcs the staff left comment/notify you or check 등업리턴 and search for your application.
