#MedTwitter im seeing multiple cases of SEVERE hypercapnia from #COVID19 out of proportion to hypoxic respiratory failure. either they are making TONS of CO2 or just NOT VENTILATING. How are people managing it? @gbosslet @laxswamy

Please help. I know vents, but im flummoxed

I have one pt whose co2 went to 160(!) WHILE ON 16L of MV.

Of course to accommodate MV I drop Paw and then lose recruited lung so P/F drops a bit. But the pO2 is still in hundreds.

So ya, O2 exchange is bad, but CO2 exchange is AWFUL

I haven’t seen it this bad before
