#COVIDー19 bedside care - bottle necks in the system to consider in strategic planning/operations front line experiences - IMHO

1. Protecting staff is crucial: if no provider = no one to care for patients
2. ED staff first highest risk contacts! Need appropriate PPE
3. Throughput: Bed management team critical to ensure ED/floors/ ICU movement is smooth and timely so no part of the care group is overwhelmed
4. Have dedicated airway teams with appropriate PPE/supplies to ensure safety (second highest risk group for getting infected RTs and Intubation staff/teams)
5. Nursing: backbone of care-need to protect minimize contact with sick patients-draw labs or patient interaction at scheduled times and PPE!
6. Hospital discharge planning ahead of time! Need to know things needed oxygen for home? - this takes time and companies are running out of equipment/supplemental O2.
7. Partner with LTACs/SNF/Acute and Subacute Rehabs - to prevent backlog and open beds for critical patients
8. If your staff can do work over a computer then do not put them at risk of exposure ( work from home or computer rounding)
9. Reduce document burden: consider scribe support or templates - let’s be real we need to be spending time saving lives right now and not writing notes!
10. Need to have clinical providers dedicated to calling families to update at home regularly:consider no visitor policy to prevent transmission to healthy family members and staff alike!
11. Palliative care more important then ever-early interactions needed
12. Hospitals/Leaders take time to go thank your staff-trust me they appreciate it. They are working hard. They are anxious and scared. They have families! All they need to know is that you care and valuethem
13. Leaders show up on the units - I am talking to you CEOs/Administrators/Division leaders-go see the trenches and let your teams know you stand with them. Email thank you is not enough!
14. We all need food and hydration create safe venues for this with healthy options
15. Accommodation for staff by partnering with local hotels ets to provide options for those that need/sick to isolate
16. Supplies in each unit: need dedicated team to ensure that supplies are available example: IV tubing, CVC kits etc
17. Encourage creative thinking and appreciate leaders in the trenches!
18. Effective, clear and frequent communication is key to avoiding errors and misunderstanding - we all need to be on the same page and same mission all the time!
