Conversation, research, policy and action now need to turn to “safely back-to-work.”
So far most plans are hand-waving without real - and realistic American - specifics.
Smart cities and states will come out with powerful plans over next 2 weeks.
Faster = better. (1/n)

Obvious:
0. Can’t think back-to-work until hospitals are ready and there is a clear downward trajectory in cases.
This means continued discipline on shelter-in-place, PPE supply chain improvements, and ideally stronger mask guidelines.
Then back to work and school via:

1. Continued safe quarantine for the vulnerable.
Actually needs to be enhanced for the long-haul in settings like nursing homes.

1b. Cautious, phased release of shelter-in-place for the young and healthy by geo.
Need guidelines for what opens when (eg daytime work before bars, etc.)

1c. Extra cautious release of inter-geo travel restrictions based on global progress.

2. Universal surgical mask mandates with fines + state-provided surgical masks.
Homemade masks help at population level but not nearly good enough.
Surgical mask provision should start with essential workers.
Need data-based guidelines for re-use and decontamination. (/n)

3. Antibody (Ab) testing for healthcare workers to protect those at the front lines, with only Ab positive folks serving in COVID areas and situations (eg EMT, contact tracers, nursing homes) if possible. (Not perfect, but better than current state.)

That’s the easy part. Then the hard part:
4) Widespread random testing for virus and antibodies in public locations (eg grocery) prob via medical vans
4b) Temp checks in public locations - even if just to keep collective consciousness of the danger high
Combined w/... (/n)

5) Immediate mandatory quarantine for the sick
5b) contact tracing w/ second order quarantine (very hard!)
5c) surveillance system of some kind to provide transparency on compliance (very hard!)

4 and 5 above are extremely counter-cultural for America and our love of what seems like freedom.
Given low suspected herd-immunity they are probably necessary to prevent a very painful second wave in which the nature of actual unfreedom will be apparent to all. (/n)

Proving out w/ science & coming to terms w/ this “probably necessary” point will be the single biggest test of our leaders and of our collective integrity.
Otherwise, in painful fits & starts, we’ll need to wait for a treatment that reduces fatality rates or for a vaccine (/n)

Cities and states - supported by corporations and volunteers of all types - will need to take this into their own hands if the near past teaches lessons for the near future. (/n)
Our public figures - particularly the star community at large - need to come together support this via influence and a unified message of togetherness and safety. (/n)
The combination of incompetence, ego, herd-thinking & cowardice in a small but critical number of seats of power has cost us so much. (/n)
But I have to believe that our leaders have it in them to be competent, selfless independent-minded and courageous
...and were just caught on their heels. (/n)

Now we have a second chance to get it right.
Let’s go.
