Sackett (1996) introduced the concept of EBP as using the latest evidence to inform decisions in patient care. Sackett’s original definition was the 1st definition of EBP and was held up as the best way to use evidence in practice as medical professionals. #PaedsRock #CYPStNN
Melnyk & Fineout-Overholt (2011) proposed an alternative to this seminal definition, making the original definition more relevant to healthcare, to better reflect the changing helathcare. They also demonstrated the 3 core principles in which EBP sits atop. #PaedsRock #CYPStNN
This can be though of as a stool, without either of the three legs (Best available practice, professional expertise and the needs and values of the client) the stool (or concept of EBP) falls down. #PaedsRock #CYPStNN
The original definition was missing the section on the needs and preferences of the client which is not patient centred care. There were countless examples of EBP falling foul of excluding the patient and as such the implementation of it being less effective. # PaedsRock #CYPStNN
There is a hierarchy of evidence showing the strength different types of EBP to be more generalisable (usable to the greatest number of people beyond the study). Note types of quantitative evidence reside higher on the hierarchy than the qualitative. #PaedsRock #CYPStNN
A flaw with the hierarchy is that it places more value on quantitative evidence that qualitative. Just because evidence isn’t as generalisable it does not mean that it’s not worthwhile! Many nursing sources need to be qualitative to be guided by our patients! #PaedsRock #CYPStNN
Shackman (2010) proposed an alternative to the hierarchy as a continuum. This shows all evidence has equal value, with qualitative and quantitative evidence displayed as a scale which still reflects generalisability. #PaedsRock #CYPStNN
Evidence based practice IS NOT JUST people sitting round talking about complex topics. We all have a role to play in EBP! #PaedsRock #CYPStNN
EBP shouldn’t just be an educational or academic principle, instead it takes all people to engage and play an active role in the nursing. Clinical trials are constantly running in practice and chances are we all come across it at some stage in our training #PaedsRock #CYPStNN
You don’t need to be a research nurse to actively implement change or EBP, in fact a lot of the responsibility lies with the nurses implementing the EBP on the front line. #PaedsRock #CYPStNN
