① E-Sign SBA’s #PPP Borrower Application Form ( https://home.treasury.gov/system/files/136/Paycheck-Protection-Program-Application-3-30-2020-v3.pdf)
② E-Sign bank's required forms ( https://www.firstrepublic.com/-/media/frb/documents/sba/sba-ppp-first-republic-client-attestation-form.pdf)
③ Gather additional required docs
④ Email or upload information online ( https://www.firstrepublic.com/resource/sba-ppp)

Under ③, this is what docs you should gather:
• Calculate average monthly payroll costs (See final regs pages 8-11, https://home.treasury.gov/system/files/136/PPP--IFRN%20FINAL.pdf#page=8).
• Support for FTE (Gusto/Paychex/ADP)
• Certificate of Incorporation
• IRS Forms 941 (quarterly reports) -or- IRS Form 940 (FY2019)
