•drvr's license # / alien reg. # and expiry
•employers ~18 mo: company, supervisor, address, phone #
•Federal Employer Identification Number (FEIN) (see W-2)
•unemployment reason
•wages and freq. of $$
•DD214 form, if military
Until stimulus passes the house, until more clarity, we should *not* be listing our 1099 clients as "employers", as it will likely trigger a Tax Audit because no payroll taxes were withheld from your income or paid by the employer and the IRS will want to look into that.
The day to apply un-employement depends on last name:

"Last names starting w/ A-F, file on Monday. Last names starting with G-N, file on Tuesday. Last names starting with O-Z, file on Wednesday. (...) all claims are effective on the Monday of the week in which they are filed."
Also the Freelancers Union started a Fund "with generous gifts from our partners at Fiverr, Upwork, Wethos, and http://AND.CO .“ Currently soliciting donations.
