Thinking about using November as a Doing Death month.
Could we make November Resolutions? Things like:

đź’€Use the D-words for these 30 days: say dying, death, dead instead of seriously ill, the end,passed. Just to see how it feels. Does it make a difference to conversations?

đź’€Reflect and plan. What kind of funeral would I like? Who knows my wishes? How can I ease the burden of arrangements for my loved ones? How about writing a list this November. Then updating it every November until - well, until you die?

đź’€Appoint/update a health proxy. Who would speak for me if I was temporarily or permanently unable to speak for myself? Do they know my wishes? Are they clear about the details? Why not have a November 'health-check' on your spokesperson's understanding?

đź’€Depending on where you live, there are probably laws that allow you to declare specific decisions to decline particular medical treatments. Are your wishes clear? Are they in writing? Remember you can only refuse, not demand, specific medical treatments.
