And Pete @PeteGordon68 the software on #Trak called #WardView which is meant to enable the Ward MDT doesn’t work at all so the patient ‘journey’ is often going around in circles

My main comment when I do a Structured Judgement Review of a Death is that the notes are chaotic and so extensive it’s impossible to grasp the whole story - same in the real workplace - we cannot see the words for the words @j59dd

And Trusts struggle to hit 4hr targets!

If downstream processes are so treacly slow everything backs up

Eg we cannot Process a Discharge as quick as 4hr because #IDL software so treacly slow and doesn’t link with Pharmacy system ...

As a PRHO in 1980s the pressure was lots of patients eg I saw over 20 cases in my first 24h on take. We had lightweight paperwork. Now staff feel the main pressures are paperwork and Computerworld and seeing the patients and deciding what to do is the easy bit.
