First of all, I can assure you QA reported A LOT of those issues. But if the game has to release and knowing is a game for the long run, things get prioritized 1/8

This means, that some bugs are moved to a later date but they are in the database.

Second. People don't really get QA. As much as they can do with their superpowers, they can only do so much in games with massive scopes.

I can tell you this NEVER HAPPENS. But just for the sake of the exercise bear with me.
200 people at 50 hours is 10000 hours a week.
10000 x 52 weeks is 520.000 hours a year. And this considers NO HOLIDAYS.
520.000 x 5 years is 2.600.000 hours of testing in total. 4/8

This means that when the game comes out and considering the IP, the production value and the marketing they could expect probably around 3 to 5 million copies sold in the first week across all platforms. So if day 1 they sell let's say 2 million. 5/8
In less than 2 hours from release they'll have more gameplay done by the users than 200 QA ever did in 5 years of overtime with no holidays. Less than 2 hours.
Things will start popping that no one could have imagined and other things were known, there was just no time to fix 6/8

People will say "they they should have delayed it". Well, it's not that simple either.
Money don't grow on trees and keeping a full development team working for multiple months can be a huge investment.

Having said all this, understand that some games need time to lift off. You can argue as much as you want that a full priced AAA game shouldn't, but games have become way too complex nowadays.
