the new form, like the interim final rule, provides no new guidance about application of affiliation rules BUT

the new form does helpfully clear up some confusing language about who needs to answer certain questions and required responses

some big changes!

- no more requirement for 20% owners to initial questions

- no more u.s. citizen/legal resident question
